Five fields per item
Description — make, model and finish, not “television”.
Purchase date — approximate is fine and a year is better than nothing.
Cost — what you paid, which is different from what replacement costs today.
Serial number — the field that separates a documented claim from an asserted one.
Photograph — one showing the item in place, and one close enough to read the label.
Nobody expects receipts for everything. What receipts do is settle the individual items where the amount is worth arguing about, which in most households is 20 or 30 objects rather than everything you own.

Video is faster than a list and worth more
Walk each room with a phone recording, narrating as you go: what the item is, roughly when it was bought, roughly what it cost. Open drawers and cupboards. Zoom in on model and serial plates.
A 20-minute walkthrough captures more usable evidence than an afternoon of typing, and it captures the things a written list always misses, which are the ordinary contents of storage.
Then do the written list only for the items worth individual argument. That is the 80% of the value for 20% of the work, and it is why most inventories that get finished are video-first.

Photograph the appraisal alongside the item, and re-date it every 3 to 5 years. A valuation that cannot be tied to a specific object is a valuation of nothing.
Grading serves the same function in collectibles, which what a grading slab certifies sets out — a third party’s dated opinion is worth more than the owner’s.

The record must not live where the loss happens
An inventory stored only in the house it documents is destroyed by the event it exists for. That is the single most common failure in this whole exercise.
Keep the working copy in cloud storage or email it to yourself, so it survives the building. Keep the paper originals that are slow to reissue in a rated box, and know that the box protects them from fire rather than from the claim taking months.
Update it once a year and after anything expensive arrives. An inventory 3 years out of date is still worth far more than none.
